    1. It has been over tightened, or kinda mangled from riding loose. It may be a persistent issue tbh. Tapered cranks have a propensity of failing once over tightened or allowed to wobble. May be time for a new crank set honestly.

      The square hole in your crank is pretty rounded out now.

      Even if you were to reinstall it with the crank bolt, it would inevitably come loose again.

      You’ll probably need to get a new non-drive-side crank arm. They usually sell the square taper ones as a single for this exact case.

      When you get the new arm, you should torque the crank bolt to ~40Nm which is a decently hefty amount of torque. If the bolt isn’t tight enough, this same scenario will happen with the new crank arm.
