First picture is the shifter with the piece missing and the second is the other where it has come loose. I don’t know what this piece is called and whether it can be replaced so can anybody here help point me in the right direction? Is this replaceable myself or would a shop have the right spares?

Probably Y0C598020. Google your exact shifter model + exploded view
Shops will be very unlikely to have one, but may be able to order.
Alternatively looking for the model number under the hood should get you to the correct page in si.shimano and the exploded diagram with spares listed
I’ve also lost mine, but because it was loose and rattled for a long time, the threads are completely wrecked.
I would have thought being able to buy a new part would be quite difficult to source, but coming across a crash damaged shifter and salvaging the part may be more likely.
Identify which shifter you have, then find a Shimano document that shows an exploded view with part numbers.
See if that document shows part numbers for the parts you need.
If you find part numbers, then see if you can find those for sale. If you can’t, then see if a Shimano dealer can order them for you.
