Crowns bent = toast. Even it was straightened on a jig it’ll never ride right.

If metal has been bent in such a way that it is permanently deformed, it no longer has the same strength (Google yield point). Do you want a fork with less strength?

If the fork is overbuilt (aka not super thin walled and light) it might hold, but never to the same standard it once did.

Find your fork geometry with this, and measure the steerer tube on your fork, etc
Check cheap old road bikes in your area and compare the fork geometry to yours, note any change in dimensions on the new fork would change your bikes handling.

I think the crown and the steerer are bent, I’d bet one of the blades is bent too. New fork time
